Salary Details

This is a full-time permanent role offering £31,081 to £33,665 per year depending on experience. The trust provides NHS pension, training, and other benefits.

About the Role

This role is all about reaching people before they slip through the cracks. Based in Hillingdon, including high-need areas like Heathrow, you’ll be supporting rough sleepers or those at risk of it. You’ll assess their needs, help them access drug and alcohol treatment, and make sure they’re linked into GP and healthcare services.

It’s a hands-on role that mixes clinical duties, street-level outreach, and partnership work with criminal justice services. You’ll be supported by senior drug and alcohol practitioners while managing a caseload of low to medium complexity. Key Responsibilities

  • Carry out assessments and help build care plans tailored to each client
  • Support individuals in registering with a GP and accessing health services
  • Deliver harm minimisation advice and motivational interventions
  • Act as keyworker for a caseload under supervision
  • Liaise with partner agencies and criminal justice teams for joint support
  • Use outcome monitoring tools to track progress and improve care
  • Maintain accurate electronic records
