Requirements

  • Understanding of mental health support and its day-to-day impact
  • Level 4 Advanced Care Support Certificate or equivalent
  • Good grasp of physical health checks such as blood pressure and BMI
  • At least 2 years’ experience in a care or support worker role
  • Interest in health promotion through lifestyle guidance and holistic assessments
  • Nice to have: Phlebotomy skills and mental health experience

About the Role

This is a hands-on role for someone who’s keen to make a real difference in both physical and mental health care. You’ll work closely with community mental health teams and local GP surgeries, offering annual physical health checks to individuals with serious mental illness. Expect a blended care approach, combining clinical measurements like blood pressure, BMI, and venepuncture with lifestyle assessments and wellbeing planning.

Key Responsibilities

  • Deliver annual physical health checks to patients with SMI including vital signs, BMI, and blood tests
  • Carry out holistic assessments covering diet, activity, substance use, and overall lifestyle
  • Offer advice and signposting that supports long-term physical wellbeing
  • Work in tandem with GPs and mental health teams to create joined-up care plans
  • Keep detailed records and support person-centred reviews of care needs
  • Be an active part of the team working on early intervention and preventative care
