Requirements
- A degree or equivalent experience in health, care, or behavioural support
- Experience supporting patients in health or social care settings
- Confident communication with the public and professionals at all levels
- Familiarity with Microsoft Office and maintaining accurate client records
- Strong organisation, group facilitation, and motivational skills
- Ability to work autonomously and travel regularly within Gwent
Desirable
- Health promotion or counselling certification
- Knowledge of behaviour change methods like MECC
- Welsh speaker or willingness to learn
- Experience with NHS tools such as CWS and relevant reporting databases
Salary Details
This Band 5 role offers a salary of 29,970 to 36,483 per year, aligned with the Agenda for Change pay scheme.
About the Role
As a Behaviour Change Practitioner, you won’t just support individuals—you’ll shift behaviours at scale. Working across community and clinical spaces, you’ll empower people to quit smoking and make healthier life choices in diet, movement, and alcohol use.
This role blends direct client support with wider community engagement. You’ll lead sessions in local areas, deliver training to fellow professionals, and offer behavioural advice that feeds into broader health strategies. Think of it as frontline wellbeing work backed by evidence, empathy, and a clear impact on local lives.
It’s not nine-to-five. It’s person-to-person. The role requires some evening and weekend work, but it’s flexible, purpose-driven, and deeply rewarding.
Key Responsibilities
- Deliver one-to-one and group behaviour change sessions across Gwent
- Use evidence-based tools to support smoking cessation and lifestyle change
- Collaborate with NHS teams to embed health improvement in everyday care
- Train and mentor professionals on behavioural science approaches
- Gather and interpret client data to shape future service improvements
- Support local health campaigns and community engagement efforts
