Salary Details
Band 4, Agenda for Change pay scheme, ranging from £27,485 to £30,162 per year depending on experience.
About the Role
This role is all about making a difference in the community. As part of the Taunton and West Somerset district nursing team, the practitioner will support patients in their own homes, providing planned care and treatments that truly impact daily lives. They’ll go beyond the responsibilities of a healthcare support worker, using their deeper understanding of health and wellbeing to provide thoughtful, tailored support.
The practitioner will deliver nursing care delegated by a registered nurse, from physical care to health-promoting activities. They’ll also handle reassessments like Waterlow, MUST, and falls assessments, ensuring patients stay safe and supported. It’s a role that blends autonomy with teamwork and offers daily opportunities to see the impact of care first-hand.
Key Responsibilities
- Deliver planned nursing care in community settings to improve wellbeing
- Work closely with registered nurses to provide tailored patient support
- Carry out specific clinical tasks within set competencies
- Reassess and monitor patient needs to maintain safety and comfort
- Promote independence and dignity through every aspect of care
