Requirements
- Current registration as a Mental Health Nurse or Social Worker in the UK
- Previous community care coordination experience
- Completed preceptorship and ability to manage own caseload
- Skilled in assessing risk, planning care and managing complex cases
- Confident in providing psychoeducation around psychosis
- Understanding of CBT approaches and medication management
- Ability to supervise students and support junior staff positively
- Strong communication skills with a sensitive and patient-centred approach

This Band 6 post is offered at £38,682 to £46,580 per annum on Agenda for Change.
About the Role
This is more than a short-term contract. As a Care Coordinator in the ESTEP team, you’ll walk alongside people at one of the most vulnerable points in their lives. Over months of engagement, you’ll build therapeutic relationships, support families and tailor care plans that reflect each person’s unique journey. From managing caseloads to coordinating MDT input, you’ll balance evidence-based practice with genuine human connection. Every risk assessment, every plan, every conversation matters because it shapes recovery.
Key Responsibilities
- Manage a caseload in line with CPA procedures so care is consistent and safe
- Develop person-centred care plans that support recovery and independence
- Assess risk and put in place tailored management strategies that protect wellbeing
- Lead on safeguarding concerns to ensure patients and families are protected
- Provide supervision for junior staff and mentor students to strengthen the workforce
- Monitor physical health and promote overall wellbeing alongside mental health support
- Contribute to MDT planning, training and service development so services evolve
- Deliver NICE-aligned interventions to give patients access to proven treatments
