Requirements
- Registered General Nurse (RGN) with current NMC registration
- Strong communication skills to work effectively with patients and colleagues
- Ability to work flexibly across a seven-day, 24-hour rota
- Good IT skills with confidence in Microsoft Office and Outlook
- Commitment to patient-centred care with enthusiasm for working in the community
Desirable:
- V100 prescribing or equivalent qualification
- Teaching or assessor experience in clinical practice
- Knowledge of SystmOne and managing patients with long-term health conditions
Salary Details
The salary is Band 5, ranging from £31,049 to £37,796 per annum, pro rata, in line with NHS Agenda for Change terms.
About the Role
This role offers the chance to be part of a dynamic community nursing team, delivering high-quality unplanned care directly to patients in their homes. You’ll assess needs, provide timely interventions, and support recovery in a way that helps people stay out of hospital. It’s a varied position where no two days look the same, and you’ll be supported with agile technology to deliver effective care across Bradford and Keighley. Whether you’re newly qualified or experienced, you’ll find the backing to build confidence, expand your skills, and contribute to service development.
Key Responsibilities
- Provide timely assessments to manage acute health issues at home and avoid unnecessary hospital admissions
- Support early discharge by delivering follow-up care that helps patients recover in their own environment
- Work collaboratively with multidisciplinary teams to secure the best outcomes for patients
- Deliver holistic care that focuses on patient experience as much as clinical outcomes
- Contribute to service improvement by sharing ideas and embracing innovation
