Requirements

• Registered General Nurse (RGN) with current NMC registration
• Strong communication skills to work effectively with patients and colleagues
• Ability to work flexibly across a seven-day, 24-hour rota
• Good IT skills with confidence in Microsoft Office and Outlook
• Commitment to patient-centred care with enthusiasm for working in the community

Desirable:

• V100 prescribing or equivalent qualification
• Teaching or assessor experience in clinical practice
• Knowledge of SystmOne and managing patients with long-term health conditions

Salary Details

The salary is Band 5, ranging from £31,049 to £37,796 per annum, pro rata, in line with NHS Agenda for Change terms.

Key Responsibilities

• Provide timely assessments to manage acute health issues at home and avoid unnecessary hospital admissions
• Support early discharge by delivering follow-up care that helps patients recover in their own environment
• Work collaboratively with multidisciplinary teams to secure the best outcomes for patients
• Deliver holistic care that focuses on patient experience as much as clinical outcomes
• Contribute to service improvement by sharing ideas and embracing innovation
