  • NVQ Level 3 in Care or equivalent skills through professional experience
  • Previous experience working in mental health or social care environments

Salary Details

This post is offered at Band 4 with a salary range of £27,485 to £30,162 per year, dependent on experience and aligned with NHS Agenda for Change pay scales.

About the Role

The Community Support Worker will join the Intensive Support Team based at St Anne’s Centre in St Leonards on Sea. The role is focused on helping service users stay well in the community, reduce hospital admissions and support smooth transitions after discharge.

Every day brings a mix of practical support, care planning and close collaboration with neighbourhood and inpatient teams. They’ll work with older adults facing age-related mental health challenges as well as adults of all ages living with dementia. The role is about more than care: it’s about empowering independence, connecting people to community resources and ensuring continuity of support that truly changes lives.

Key Responsibilities

  • Promote independence in service users so they can thrive in their own communities
  • Provide practical and regular support to improve daily living and wellbeing
  • Help service users access welfare benefits and community resources for stability
  • Link people to local groups and day centres, reducing isolation and improving social support
  • Ensure care plans are followed through, keeping communication clear with all involved agencies
  • Monitor quality of care and seek feedback to make sure services meet real needs
  • Maintain accurate records to support consistent and safe care delivery
  • Take part in supervision and training to build confidence and professional skills
