Requirements
- Core professional qualification in nursing, social work or occupational therapy
- Knowledge of the Care Programme Approach and risk management principles
- Experience supervising students and engaging in clinical or managerial supervision
- Strong understanding of safeguarding children and vulnerable adults
- Ability to work with service users with complex mental health needs in a collaborative way
- A flexible, team-focused approach with good communication skills

About the Role
This Care Coordinator role puts you at the heart of recovery-focused work with people experiencing psychosis. You’ll oversee the clinical management of a caseload, working with service users and their families to develop care plans that support independence and wellbeing.
The team covers the north of Manchester but also contributes to wider service needs, so you’ll sometimes travel across the city or to inpatient units. No two days are the same. One day you might be completing a social care act assessment, the next leading on risk planning or advising partner agencies about mental health care.
You’ll join a passionate, multidisciplinary team who value professional development and peer support. It’s a role that demands flexibility and resilience, but the impact you’ll make in helping people reclaim their lives is profound.
Key Responsibilities
- Manage a personal caseload of service users aged 14–65, ensuring recovery-focused care
- Carry out social care act assessments and contribute to funding panel decisions
- Take part in duty rotas and assessments, helping the service respond quickly to needs
- Support clients to engage with treatment and develop self-help strategies to boost independence
- Promote health awareness and provide guidance to community services and partner agencies
- Contribute to safeguarding investigations, protecting vulnerable adults and children
- Undertake specialist assessments, including capacity assessments under the Mental Capacity Act
- Complete and review risk assessments, putting management plans in place to reduce harm
