Requirements
- Registered qualification in Social Work
- Registration with Social Work England
- Experience supporting people with palliative care needs, long-term illness, or cancer
- Strong communication skills with families and interdisciplinary teams
- Knowledge of safeguarding, MCA, DOLs, and relevant legislation
- Confidence in managing complex caseloads independently
- Self-resilience to work in challenging and sensitive situations
Salary Details
This Band 7 post offers between £56,276 and £63,176 per year, with NHS benefits and career progression built in.
About the Role
This is more than a social work post. It’s about standing alongside patients and families in Camden and Islington as they navigate the toughest times. The successful candidate will work as part of a thriving community palliative care team, offering specialist social work input around illness, loss, and bereavement.
Day to day, they’ll assess needs, plan support, and manage caseloads where every decision matters. They’ll link with other professionals to ensure families receive joined-up care, while acting as a trusted resource for colleagues within and outside the Trust. The impact stretches beyond patients to families before and after bereavement, shaping how people experience their final months and how loved ones carry on afterward.
Key Responsibilities
- Provide holistic social work support for patients and families facing life-limiting illness
- Deliver bereavement and grief support, helping families cope before and after death
- Collaborate with nurses, doctors, and allied professionals to ensure joined-up care
- Act as a specialist resource, sharing expertise through teaching and consultation
- Manage and prioritise a diverse caseload with compassion and professionalism
- Advocate for patients and families, ensuring dignity and choice are at the centre
