About the Role

You’ll assess, plan, and deliver patient-focused care across treatment pathways, always with an eye on promoting long-term health and recovery. From triaging patients and managing medicines to supporting discharge planning, your role will be crucial in ensuring continuity of care.

What sets this apart is the collaborative nature of the work. You’ll partner with officers, governors, and other healthcare professionals to deliver joined-up treatment plans that really work. The difference you make here extends beyond health – you’ll be helping people find stability and inclusion, shaping safer communities in the process.
