Requirements

  • Registered Nurse with current UK registration in Mental Health, LD, or Adult nursing
  • Completion of Mentorship Course or ENB equivalent
  • Evidence of continuous professional development
  • Significant post-qualifying experience in a relevant care group
  • Strong background in clinical assessments including risk assessments
  • Experience working with people with complex health needs
  • Skilled in supervising staff and students
  • Proven ability to work consultatively with families and professionals

About the Role

This community mental health nurse position focuses on triaging referrals for older adults across West Sussex. By screening and guiding clinical pathways, you’ll make sure patients get the right care, right away. Day to day, you’ll work closely with service users, carers, and partner agencies, helping shape tailored care plans that support safety and wellbeing. Your input matters because you’ll be the first point of contact in many cases, setting the tone for compassionate care and effective support.

Key Responsibilities

  • Screen referrals to determine safe and effective clinical pathways
  • Provide specialist nursing advice that shapes patient outcomes
