Requirements

  • Registered Learning Disability or Mental Health Nurse (RNLD/RMN) with current UK registration
  • Post-registration experience supporting individuals with learning disabilities
  • Knowledge of SEND frameworks, care planning, and transition pathways
  • Ability to perform comprehensive health assessments and develop care plans
  • Comfortable working autonomously while collaborating within a multi-disciplinary team
  • Skilled in communication, organisation, and digital documentation
  • Enthusiasm for helping young people and families navigate complex transitions

Salary Details

This role is offered under the Agenda for Change Band 6 pay scheme, between £44,485 and £52,521 per year, including London allowances.

About the Role

This 12-month fixed-term position gives you the chance to make a visible difference in the lives of young people with learning disabilities as they move from children’s to adult services.

As a Community Learning Disability Nurse, you’ll work in the Bromley Community Learning Disability Team within the Transition Nursing group. Your goal is to help young people and their families feel supported during a time that can often feel uncertain. You’ll manage your own caseload, deliver tailored health and risk assessments, and coordinate multi-agency input to build pathways that work.

You’ll also run health promotion sessions, mentor junior staff, and guide families through SEND processes. Every decision you make and every conversation you lead will directly shape how these young people experience independence and access to care.

Key Responsibilities

  • Manage a caseload of young people in transition, providing individualised health and wellbeing plans that build confidence and stability
  • Work collaboratively with social care, education, and healthcare partners to deliver joined-up support
  • Conduct detailed health assessments that inform safe, person-centred transition planning
  • Facilitate health education sessions for families and carers to promote understanding and resilience
  • Advocate for service users to ensure equal access to mainstream services and opportunities
  • Mentor students and junior staff to develop future nursing leaders in learning disability care
  • Contribute to audits and evaluations to improve service quality and outcomes across the Trust
