Requirements
- Professional Social Work qualification (e.g. DipSW, BA, or MA in Social Work)
- Registration with Social Work England
- Knowledge of social care legislation, including the Care Act 2014
- Experience in assessments, care planning, and multidisciplinary collaboration
- Strong communication skills for building trust with service users and carers

This Band 5 position offers £32,602 to £39,686 a year (including 5% HCAS), under the Agenda for Change pay scheme.
About the Role
As an ASYE Social Worker, you’ll join the Adult Community Mental Health Service (ACMHS) in Southwest Hertfordshire, providing recovery-focused social care to adults living with mental ill health. You’ll work as part of a multidisciplinary team, ensuring every service user receives the right care, at the right time, in the right way.
Your role goes beyond paperwork. You’ll assess needs, design care plans, and coordinate support that promotes independence and dignity. You’ll help people navigate adversity and regain control over their lives, while ensuring their safety through regular risk assessments and safeguarding practices. Every decision you make will have a visible impact on someone’s recovery journey.
Key Responsibilities
- Carry out social care assessments and reviews for adults with complex mental health needs to ensure timely and appropriate support
- Coordinate care plans that prioritise recovery, independence, and safety for each individual
- Work with carers, families, and partner organisations to deliver joined-up care and shared understanding
- Uphold safeguarding responsibilities for both adults and children in line with statutory policy
- Champion equality and challenge discrimination to ensure fair treatment for all service users
- Maintain professional development through reflection, supervision, and adherence to Social Work England standards
