Requirements
- Level 3 healthcare qualification or equivalent experience that shows real hands on skill
- Care certificate to support safe practice
- Literacy and numeracy at Level 1 so documentation feels manageable
- Experience in health or social care where you’ve helped people through daily tasks
- Calm communication skills that help patients and families feel heard
- Basic IT confidence for simple clinical systems

This Band 3 role offers 24937 to 26598 pro rata depending on experience.
About the Role
You’ll join the Hospital at Home team where the goal is simple help people stay safe in their own home while receiving clinical support. Some days you’re taking bloods and checking on symptoms. Other days you’re reassuring a family who needs a steady voice. The pace shifts but the intent stays the same keeping people stable while avoiding unnecessary hospital stays. If you enjoy hands on patient care with a mix of community and acute work you’ll feel right at home. You’ll use your practical skills like venepuncture and ECG recording while learning more about remote monitoring. It’s the kind of role where small actions have huge impact and patients often remember your kindness long after the visit.
Key Responsibilities
- Carry out clinical tasks like blood samples and ECGs so the team can act on real time information
- Support remote monitoring to help patients stay safe between visits
- Observe changes in health and share concerns early so issues don’t snowball
- Visit patients at home to check progress and offer a reassuring presence
- Work with the wider team so everyone stays on the same page
