About the Role
This rotational post gives the therapist room to grow. Each rotation offers new challenges that sharpen judgement and build confidence. The therapist will support people with complex health and social care needs through assessments, goal setting and hands on interventions. They will help people regain independence, make informed choices and move toward safer outcomes at home or in the hospital. The work matters because every small step changes the way someone lives their day.
The role involves close partnership with families and multidisciplinary teams. They will monitor progress, adjust plans and offer practical solutions. They will also support conversations during difficult moments and help people understand what to expect next. With each rotation the therapist will see how services connect across settings and why this integration matters for patients who rely on coordinated care.
Key Responsibilities
- Assess needs and design clear treatment plans so patients understand their next steps
- Deliver one to one or group interventions that support safer mobility and daily routines
- Review progress and adjust recommendations to keep patients moving toward their goals
- Recommend equipment that improves comfort and independence
- Document assessments and outcomes to support continuity of care
- Work with multidisciplinary teams to guide shared decisions
- Support unqualified staff through supervision to keep workflows safe
- Contribute to audits and service improvements that strengthen patient experience
