- Experience supporting people with advanced illness in community settings

Salary Details
The role offers £47,810 to £54,710 yearly based on NHS Band 7.
About the Role
This position sits within the community specialist palliative care team where you guide people with advanced or terminal illness and help families feel supported. You link closely with GPs, district nurses and local hospices to keep care consistent. You use your clinical judgement to assess patients directly, shape care plans and give specialist advice that reduces distress. The team is working toward a seven day service so flexibility helps. With ongoing pilot changes around neighbourhood models you may see the team shape evolve but the heart of the work stays the same. Key Responsibilities
- Provide specialist assessments that help patients stay comfortable in their preferred setting
- Guide families through difficult moments so they feel informed and less overwhelmed
- Work jointly with GPs and consultants to keep care joined up
- Offer expert advice to primary care teams so patients receive steady and safe support
- Attend key meetings and contribute to service improvement so standards rise over time
- Help review policies and clinical processes so the service stays high quality
