Job Description
The LYPFT Learning Disability service provides multi-professional, specialist health services for adults with learning disabilities. These services are delivered across community settings, respite care, and the intensive support team. LYPFT collaborates with key stakeholders, including service users, carers, adult social care, third-sector organizations, and private providers to offer comprehensive healthcare to individuals with complex health needs.
An exciting opportunity has arisen for a Community Nurse to join the East North East Community Learning Disability Team on a permanent basis. The successful candidate will support adults with learning disabilities who require specialized health care due to vulnerabilities, complex physical health, sensory, communication, and behavioral impairments.
Responsibilities
Responsibilities include providing direct clinical care, developing intervention toolkits for nurses, and managing a caseload. The role involves preventing hospital admissions by ensuring appropriate care plans are in place. The Community Nurse will also provide clinical and operational supervision to junior colleagues and contribute to service development projects.
Skills and Qualifications
- Registered Nurse with relevant experience
- Strong clinical skills in providing care to adults with learning disabilities
- Ability to carry out comprehensive, person-centered holistic assessments
- Experience in managing a caseload and providing operational supervision
- Passion for improving the health and well-being of adults with learning disabilities
- Ability to coordinate across multi-disciplinary and multi-agency teams
- Commitment to promoting social inclusion and working in a diverse population
- Understanding of safeguarding principles
- Experience in time management and delivering the nursing process effectively
Compensation
- Salary: £35,392 – £42,618 per year
- Part-time position
- Monday to Friday schedule
