Job Description:

The Leeds and York Partnership NHS Foundation Trust (LYPFT) is seeking compassionate and experienced Support Workers to join their team. This role involves supporting individuals with learning disabilities in a supported living environment, promoting independence and helping them achieve their personal goals. The ideal candidate will have experience with Positive Behavior Support (PBS) and postural management.

Key Responsibilities:

  • Assist with daily living activities, including personal care, meal preparation, and household tasks.
  • Support individuals in accessing community activities and services.
  • Follow support plans and promote independence.
  • Provide healthcare interventions and follow positive behavior support plans.
  • Maintain accurate records and communicate effectively with team members, families, and other professionals.
  • Encourage social interaction and help individuals build relationships.
  • Offer emotional support and companionship.

Requirements:

  • Previous experience working with people with learning disabilities.
  • Experience in a supported living environment.
  • Strong communication and interpersonal skills.
  • Relevant qualifications in health and social care (e.g., Level 2/3 diploma in care or equivalent).
  • Flexibility to work shifts, including early, late, night, weekends, and public holidays.
  • A commitment to providing high-quality, person-centered care.
