Job Description Stockport Council is seeking three dedicated ARCH/Discharge Social Workers for permanent full-time positions at Stepping Hill Hospital. The role involves facilitating patient transitions back into the community, focusing on the “Home First” ethos. Responsibilities include discharge planning, working with multidisciplinary teams (MDT), supporting the Transfer of Care hub, and aiding the Discharge to Assess (D2A) bed-based provision. Candidates will also collaborate with the Reablement community team, offering Care Act assessments and support planning within the community.
