Job Description
Leeds and York Partnership NHS Foundation Trust (LYPFT) is offering an exciting full-time opportunity for a skilled Support Worker to join their Older Peoples Service Community Mental Health Team in Leeds. This role involves working with Service Users who have severe, enduring, and complex mental health needs, such as mood disorders, psychosis, and dementia. The successful candidate will work independently under supervision and contribute to the Multi-Disciplinary team by delivering comprehensive care co-ordination, developing robust care plans, and conducting risk assessments.

Skills and Qualifications

  • Mental Health Expertise: In-depth understanding of severe, enduring, and complex mental health needs.
  • Independent Work: Ability to work independently under supervision.
  • Clinical Supervision: Proficiency in clinical supervision and mentorship with a personal development plan.
  • Care Coordination: Comprehensive skills in assessing, planning, implementing, and evaluating individual care needs.
  • Technical Knowledge: Familiarity with electronic care systems.
  • Driving Requirement: Full UK driving license and access to a car.
  • Therapeutic Interventions: Ability to demonstrate a range of therapeutic interventions.

Salary Details
£28,407 – £34,581 per year
