Requirements

  • Minimum NVQ level 3 in Health or Social Care.
  • At least two years of relevant work experience.

About the Role

As a Health Care Assistant, the role involves participating in the delivery of personalized care to patients within their own homes. This includes routine and acute patient-centered interventions to prevent unnecessary hospital admissions. The role is part of the Stroud Locality Integrated Community Team (ICT), providing essential support to the community nursing teams.
