Requirements

  • Diploma in Health and Social Care level 2 or equivalent
  • Care Certificate
  • Key skills in Literacy and Numeracy level 2Effective verbal and written communication skills
  • Time management and ability to prioritize effectively
  • Right to work in the UK
  • Experience as a Band 2/3 HCA or equivalent in a care environment is desirable
  • Knowledge of person-centered care and legal frameworks including Mental Health legislation

About the Role

This role involves providing support for older adults living with dementia in the community and care homes. The candidate will work alongside a multi-disciplinary team, supporting clinicians in screening referrals, reviewing notes and documentation, and conducting face-to-face patient reviews. They will carry a small caseload and perform basic physical health checks as necessary. The role requires flexibility, adaptability, and a car driver to meet the community service needs.

Key Responsibilities
