This position offers a competitive annual salary within the range of £28,407 – £34,581, with opportunities for advancement and incremental increases based on experience and responsibilities.
About the Role
The Community Mental Health Nurse in this role will join the Dementia and Older Persons Mental Health Service, addressing the complex needs of clients within a multi-disciplinary framework. Responsibilities include managing a team caseload, conducting comprehensive assessments, creating and implementing individual care plans, and actively contributing to the development of dementia care services. They will participate in regular team meetings and provide specialist interventions in line with care pathways.
Key Responsibilities
- Manage and evaluate a team caseload with a focus on client-centered care.
- Conduct assessments and risk management planning for dementia patients.
- Participate in multi-disciplinary meetings and support duty rota coverage.
- Facilitate evidence-based individual and group interventions.
- Mentor and supervise students and junior staff as needed.
