Job Overview
An opportunity is available within Brent’s Community Nursing Teams for Band 7 District Nurse Team Leaders. Te role involves providing clinical leadership in delivering high-quality care within the community setting, as well as managing daily operations of a team.
Main Duties and Responsibilities
- Managing care for patients referred by General Practitioners (GPs), focusing on preventing unnecessary hospital admissions in alignment with commissioning objectives.
- Conducting comprehensive assessments, creating individualised care plans, and coordinating with other agencies involved in patient care.
- Communicating with GPs, participating in meetings, and planning advanced treatments to mitigate inappropriate hospital admissions.
- Demonstrating clinical credibility by delivering high standards of evidence-based care and reviewing practices to ensure clinical efficiency.
- Collaborating with the Clinical Development Lead to organise and implement training programs for District Nurses.

Education and Qualifications
Essential Criteria
- 1st level registration
- Three years of District Nursing experience at Band 6 level
- District Nurse or Teaching qualification
- Nurse prescribing qualification relevant to the role
- Evidence of continued professional development, including courses relevant to the area of work
- Evidence of team management experience
- Relevant specialist courses
Desirable Criteria
- Management qualification or willingness to work towards one
- Evidence of working towards a degree
Knowledge and Experience
Essential Criteria
- Experience as a mentor at pre- and post-registration levels
- Nurse leadership and team management experience
- Experience in appraisals, clinical supervision, and setting objectives
- Ability to manage a team and work collaboratively in a multidisciplinary setting
- Familiarity with current NHS issues, legislation, clinical governance, and risk management
- Participation in research and clinical audits
- Understanding of budgeting and performance monitoring
Desirable Criteria
- Experience in developing strategies, clinical guidelines, and policies

Essential Criteria
- Critical analysis and problem-solving skills
- Strong interpersonal, communication, and negotiation skills
