The role involves supporting service users in line with clinical pathways such as the Assertive Outreach and Dual Diagnosis pathways, as outlined in the CMHT operational procedure. Short-term care periods will also be provided to help service users develop independent living skills.
Responsibilities include offering support, advice, and practical assistance to an allocated group of service users, following care plans designed to facilitate goal planning, achievement, and recovery.
