About the Role
The Advanced Care Practitioner will be instrumental in providing first-contact care for medical and nursing conditions in care homes and community settings. Working collaboratively within a multidisciplinary team, this role emphasizes advanced clinical practice, evidence-based decision-making, and patient-centered care. Responsibilities span diagnosing and managing acute and chronic conditions, prescribing medication, and delivering holistic care plans tailored to patients’ unique needs.
Key Responsibilities
- Serve as the primary clinical contact for care homes in managing patient care.
- Utilize advanced skills in history-taking, examination, and clinical decision-making.
- Diagnose and treat acute and chronic conditions, formulating tailored management plans.
- Lead health promotion and education initiatives to enhance community well-being.
- Collaborate with multidisciplinary teams to ensure high-quality care delivery.
- Prescribe medications (if independently qualified) in line with clinical guidelines.
- Conduct advanced assessments for triaging and treatment.
- Mentor and support healthcare students and professionals within the organization.
