    • Registered Nurse Level 1 (Adult).
    • Mentor and Assessor Qualification (or equivalent).
    • Clinical skills relevant to community nursing.

    • Full UK driving license and access to a car for work.

  • About the Role:
    As part of the Swanshurst IMT, you will provide essential nursing care to patients within their own homes. The role requires working closely with a multidisciplinary team to deliver holistic assessments and care that promotes independence and well-being. Responsibilities include clinical care delivery, maintaining patient independence, and ensuring effective collaboration with team members.
  • Key Responsibilities:
    • Deliver clinical care to patients in their homes.
    • Collaborate with the wider multidisciplinary team to meet patient needs.
    • Provide holistic assessments to ensure personalized care.
    • Participate in an 8am–6pm shift pattern, transitioning to 8am–8pm within 12 months.
    • Weekend and bank holiday working included.
  • Other Details:
    The role requires a high level of mobility and the ability to travel independently. Flexible working arrangements may be discussed at the interview stage.
