As part of the Swanshurst IMT, you will provide essential nursing care to patients within their own homes. The role requires working closely with a multidisciplinary team to deliver holistic assessments and care that promotes independence and well-being. Responsibilities include clinical care delivery, maintaining patient independence, and ensuring effective collaboration with team members. - Key Responsibilities:
- Deliver clinical care to patients in their homes.
- Collaborate with the wider multidisciplinary team to meet patient needs.
- Provide holistic assessments to ensure personalized care.
- Participate in an 8am–6pm shift pattern, transitioning to 8am–8pm within 12 months.
- Weekend and bank holiday working included.
- Other Details:
