About the Role
The Cancer Support Worker team at UHBW is seeking a motivated and compassionate individual to join their established team. This role involves providing personalized support and information to individuals living with or affected by cancer. The team works closely with cancer clinical teams and services across the Trust to enhance the patient experience. This is an opportunity to contribute to meaningful care improvements within a supportive, interdisciplinary environment.

Main Duties

  • Coordinate necessary assessments, appointments, or investigations to ensure efficient service access for patients.
  • Respond promptly to sudden patient deterioration or emergencies, alerting clinical teams as needed.
  • Conduct holistic needs assessments and contribute to the development of individual care plans.
  • Make pre-planned calls to patients to assess needs, enabling a proactive approach to care.
  • Signpost patients to appropriate support services, encouraging self-management where possible.
  • Support healthy and active lifestyle choices for patients.
  • Plan and organize Health and Well-Being events or other self-management initiatives.
  • Assist with developing patient information resources, including leaflets and website content.
  • Work on service development, audits, and research projects.
  • Staff the Macmillan Centre on a rota basis, providing information and support to patients, carers, and families.

Person Specification

Knowledge and Experience
Essential:

  • Relevant health or social care experience (AfC Band 3 or equivalent).
  • Experience of multi-professional teamwork.
  • Proficiency in data management and IT systems.
  • Strong communication skills, both verbal and written.
  • Understanding of person-centered care and the health/social care environment.
  • Experience coaching or teaching patients and carers.

Desirable:

  • Knowledge of Macmillan Cancer Support and its role across the UK.
  • Experience coordinating patient workloads.
  • Awareness of national cancer policies and strategies.
  • Familiarity with cancer treatments and medical terminology.

Skills and Abilities
Essential:

  • Strong organizational and observation skills.
  • Initiative and the ability to prioritize workloads.
  • Empathy and understanding of challenges faced by cancer patients.
  • Effective teamwork and self-motivation.
  • Ability to handle complex emotional situations.

Desirable:

  • Coaching or teaching qualification.
  • Evidence of continued professional development.

Qualifications and Training
Essential:

  • GCSE English and Mathematics (Grades A-C) or equivalent.
  • NVQ4 or NVQ3 with additional diploma-level training.

Desirable:

  • Willingness to undertake Macmillan learning and development courses.

Additional Requirements

  • Ability to travel between sites.
  • A calm, mature, and professional demeanor.
