Requirements
- Registered Practitioner (Mental Health Nurse, OT, Social Worker)
- Mentorship or supervisor qualification
- Evidence of professional development since registration
- Experience of working as part of a multi-disciplinary team
- Experience of care co-ordinating within CPA framework
- Experience of working in a community setting
- Experience of mentoring or supervising students
- Excellent communication skills, both verbal and written
- Excellent IT skills
- Full driving licence (required)

About the Role
As a Care Co-ordinator (Social Worker), you will work as part of a multi-disciplinary team to assess, plan, and review health and social care needs under the Care Programme Approach (CPA) for a group of service users with a forensic/offending history. You will undertake comprehensive assessments, provide effective case management, and ensure effective discharge care planning. This role involves working in partnership with the criminal justice system and other agencies to provide high-quality primary care and mental health services.
Key Responsibilities
- Assess, plan, and review health and social care needs under the Care Programme Approach (CPA)
- Undertake comprehensive assessments alone or with colleagues
- Provide effective case management to a defined caseload
- Ensure effective discharge care planning
- Engage with people with mental health problems, assessing their needs, designing and delivering appropriate interventions
- Use supervision positively and effectively
- Work patients to increase their level of self-determination and resilience
- Ensure compliance with relevant legislation
