Requirements
- NVQ Level 3 in Health and Social Care (or equivalent)
- At least one year of experience in a mental health setting
- Excellent communication and time management skills
- Familiarity with risk assessment and care planning tools
- Comfortable working with diverse communities
- Open to learning, problem-solving, and co-producing care plans
Bonus if you have:
- A full UK driving licence
- First aid or health and safety awareness
- Knowledge of therapeutic approaches such as OT and psychology
Salary Details
£31,081 to £33,665 per year, depending on experience
About the Role
This isn’t a sit-at-your-desk kind of job. As an Outreach Support Worker, you’ll be out in the community – walking alongside people on their mental health journey. You’ll work directly with service users through 1:1 and group interventions, supporting delivery of care planned by professionals like RMNs, psychologists, and occupational therapists.
You’ll also help people reconnect with the world around them – whether it’s helping them get back into education, find work, or engage with social opportunities. Every action you take helps someone move toward stability, dignity, and a better life. If you believe in impactful work, this role delivers.
Key Responsibilities
- Support delivery of care plans and interventions designed by mental health professionals
- Help users access services and engage in their communities
- Use Dialog+ and other tools to assess health and social care needs
- Participate in team meetings and share ideas with the wider care network
- Promote recovery by supporting service users to pursue meaningful life goals
- Maintain progress notes in the electronic record system
- Provide a culturally sensitive service and communicate with compassion
