Job Description

LYPFT is seeking an experienced Harm Reduction Practitioner to join the CREST team. This role involves supporting individuals with co-occurring mental health and addiction needs, focusing on harm reduction and recovery. The successful candidate will work closely with the Multi-Disciplinary Team (MDT) to provide individualized support and develop care plans. The role includes both direct work with service users and indirect interventions through the MDT.

Skills and Qualifications

  • Experience in harm reduction and substance misuse
  • Ability to coproduce care plans and plan interventions
  • Creative thinking and engagement skills
  • Experience in working with individuals with complex rehabilitation needs
  • Ability to work independently and as part of a team
  • Strong communication and liaison skills with service users, carers, and other service providers

Compensation
