Job Overview
Mersey Care NHS Foundation Trust has Care Practitioner jobs, for qualified people to join their Out of Hours District Nursing Team. The service operates across Liverpool from 8pm to 8am, seven days a week. This role is ideal for individuals with excellent empathy, communication skills, and a person-centered approach to care. Clinical decision-making and problem-solving are vital to this position, and you will work both independently and as part of a team. Full mentorship and support will be provided to help you develop the necessary competencies to deliver evidence-based care to patients in their homes.

Qualifications
Essential criteria

  • Registered Practitioner

Desirable criteria

  • Mentor/teaching qualification or equivalent work-based experience

Knowledge/Experience

  • Clinical experience in a relevant field.

  • Clinical supervision experience

  • Knowledge of Clinical Governance, patient group directions, and associated policies.
  • Understanding of the audit process.
