The Health Care Assistant will be a vital member of the St Ives District Nursing Community Team. This role involves working closely with the nursing team to provide high-quality care to adults with complex health needs within their own homes. Responsibilities include assisting with personal care, administering medications, and supporting patients with daily living activities. The role requires a compassionate and dedicated individual who is committed to making a difference in the lives of others.
Key Responsibilities
- Assist in delivering care based on prescribed treatment plans
- Communicate effectively with patients, carers, and other healthcare professionals
- Maintain accurate and timely documentation of patient care
- Promote patient independence and self-management
- Ensure safe use and maintenance of equipment
