The Role: Support Worker in Tees Liaison Psychiatry

If you’ve got a knack for person-centered care and experience working with folks who have complex mental health needs, this could be your chance to shine.

What You’ll Be Doing:

  • Monitoring and reviewing intervention plans for patients
  • Keeping an eye on patients’ moods, behaviors, and overall wellbeing
  • Working alongside acute trust staff, showing them the ropes of skilled mental health care
  • Supporting complex discharges, teaming up with community services
  • Being a champion for person-centered, individualized care

What You’ll Need:

  • Diploma in Health and Social Care Level 3 or equivalent
  • Care Certificate
  • Literacy and Numeracy skills at Level 2
  • Previous experience as a Band 2/3 Healthcare Assistant or similar role
  • A passion for mental health care and understanding the needs of older people
  • Great communication skills and a knack for record-keeping
