The Role: Support Worker in Tees Liaison Psychiatry

If you’ve got a knack for person-centered care and experience working with folks who have complex mental health needs, this could be your chance to shine.
What You’ll Be Doing:
- Monitoring and reviewing intervention plans for patients
- Keeping an eye on patients’ moods, behaviors, and overall wellbeing
- Working alongside acute trust staff, showing them the ropes of skilled mental health care
- Supporting complex discharges, teaming up with community services
- Being a champion for person-centered, individualized care
What You’ll Need:
- Diploma in Health and Social Care Level 3 or equivalent
- Care Certificate
- Literacy and Numeracy skills at Level 2
- Previous experience as a Band 2/3 Healthcare Assistant or similar role
- A passion for mental health care and understanding the needs of older people
- Great communication skills and a knack for record-keeping
