Requirements
- Recognized professional qualification (RMN, RN Mental Health, OT, Social Worker)
- Extensive experience in mental health and risk assessment
- Ability to work both autonomously and within a multidisciplinary team
- Leadership and supervision experience
- Skilled in clinical communication and maintaining accurate records
- Knowledge of the national and local mental health and social care agenda
- Experience in working with service users, carers, and other agencies

About the Role
The Primary Care Mental Health Specialist Practitioner will play a crucial role in the ongoing development of the primary care mental health team within Leeds Mental Wellbeing Service. This role focuses on supporting individuals with unmet mental health needs, offering short-term interventions such as formulation, risk assessment, crisis planning, psychoeducation, and coping strategy work. The practitioner will collaborate with general practices, community mental health teams, psychology services, and third-sector colleagues to ensure the availability of appropriate services.
Key Responsibilities
- Provide triage, specialist mental health assessments, and brief interventions
- Offer consultation and decision support for primary care and community mental health teams
- Deliver supervision and leadership to colleagues
- Engage service users and carers in all aspects of care delivery
- Contribute to service development initiatives within the team
- Develop and maintain communication networks with other agencies
