Job Overview
An exciting opportunity is available within the Leicester Renal Community Team for a Band 5 Nurse.
A 1st level registered nurse with at least 12 months’ post-registration experience is required. Knowledge of renal nursing and dialysis is desirable.
The Renal Community Team offers a supportive environment to develop specialist nursing skills. Responsibilities include planning and delivering high-quality care for various renal patients, providing pre-dialysis education, training patients on peritoneal dialysis and home haemodialysis, supporting patients and families in the community, and reviewing patients in clinic and inpatient settings.
The Leicester Renal Community Team has visible and supportive leadership, offering guidance to ensure a seamless transition into the team. Evidence of continuous professional development within the last 12 months is essential, alongside relevant skills and aspirations.
Core working hours are Monday to Friday, between 8:00 am and 5:00 pm.
Main Duties of the Role
- Assess, plan, implement, and evaluate evidence-based nursing care for patients.
- Deliver comprehensive education programs to patients and families, enabling informed choices for kidney replacement therapy in home environments.
- Provide advice, support, and information to patients and carers within the scope of knowledge and experience.
- Collaborate closely with senior team members and contribute to managing and organising community clinical areas.
- Set and maintain high standards of nursing care.
- Participate actively in the education, development, and supervision of staff members.
- Act as a clinical role model, supporting students and learners on placement.
- Work collaboratively with professionals and agencies to address patient needs, ensuring health education and promotion.
- Organise and perform investigations in community settings, supporting staff in providing a “drop-in” renal day service.

Detailed Job Description and Responsibilities
- Conduct comprehensive assessments of patient nursing needs, planning, and delivering care according to changing requirements.
- Ensure accurate reporting and record-keeping of patient information.
- Involve patients and carers in care planning and delivery.
- Maintain high standards of infection prevention in a safe environment.
- Collaborate with other professionals to support safe discharge and ongoing care needs.
- Establish effective communication with patients and carers.
- Respond appropriately to urgent and emergency situations.
Person Specification
Essential Criteria
- One year of post-registration experience.
- Evidence of up-to-date professional development portfolios.
- Teaching and assessing qualifications.
Desirable Criteria
- Previous renal experience.
- Intermediate or advanced communication course certification.
- Clinical skills assessment qualification.
