Job Summary

An exciting opportunity has arisen for a Social Worker to join Livewell Southwest’s Adult Social Care mental health pathway. This part-time role (21 hours per week) offers a dual function, focusing on hospital discharge and community review. The successful candidate will play a key role in the organisation’s integrated health and social care model, contributing to person-centred outcomes under both the Personalised Care Framework and the Care Act (2014).

This is an internal, 18-month fixed-term role, and the Social Worker will work closely with Livewell Southwest’s Mental Health Services, applying intervention-based approaches. The role will require collaboration with other professionals in a multidisciplinary team (MDT), ensuring that the best possible outcomes are achieved for individuals in need of social care.

Key Responsibilities

  • Undertaking complex, proportionate assessments such as mental capacity assessments and health and social care assessments for individuals with varying needs.
  • Arranging and reviewing the delivery of social care support, including community-based services and interventions aimed at promoting independent living.
  • Supporting individuals to take informed risks in complex situations to ensure their personal outcomes and wishes are prioritized.
  • Conducting safeguarding investigations and making best interest decisions that may result in deprivation of liberty for individuals at risk.
  • Collaborating with healthcare professionals to deliver compassionate, high-quality healthcare and social care services.
  • Supervising and providing training to students and Band 5 colleagues to ensure safe and professional practice within the team.
  • Contributing to the induction process for new team members and delivering relevant training as needed.
  • Participating in a 7-day working model, as required, to meet the needs of the service and ensure continuous, high-quality care delivery.

Requirements

  • Degree or equivalent qualification in Social Work (e.g., DipSW, CQSW, CSS).
  • Registration with Social Work England as a Social Worker.
  • Commitment to completing the Assessed and Supported Year in Employment (ASYE) for newly qualified social workers.
  • Evidence of Continuous Professional Development (CPD), meeting the Standards of Proficiency for Social Workers.
  • Knowledge of the Care Act (2014), Health and Social Care Act (2012), Mental Capacity Act (2005), and other relevant legislation.

Desirable Experience

  • Previous experience working in a mental health setting.
  • Post-qualification award, such as Best Interest Assessor (BIA).
  • Experience with multi-disciplinary and partnership working in an adult social care setting.

This role is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975. Therefore, a disclosure to the Disclosure and Barring Service (DBS) is required to check for any previous criminal convictions.
