Requirements
- GCSEs in Maths and English
- NVQ Level 2 or 3 in Health and Social Care or strong relevant care experience
- Full UK driving licence and access to a vehicle for community visits
- Understanding of physical or mental health conditions, or the willingness to learn
- Confident with basic IT skills, especially Word and Outlook
- Physically fit enough to manage standing, lifting, and handling procedures
- Someone who lives their day-to-day with compassion, courage, and communication

About the Role

As an Assistant Practitioner in the Community Occupational Therapy Service, you’ll play a hands-on role in helping individuals regain or maintain independence in their own homes. From carrying out assessments to recommending equipment and reviewing care plans, you’ll help make life easier and safer for people dealing with physical health challenges. You won’t be on your own. You’ll be supported by a qualified clinician who’ll help guide your practice and build your confidence, especially as you support junior staff and student placements. It’s people-first work, done in real homes, often under challenging circumstances—but it’s the kind of work that sticks with you.
Key Responsibilities
- Carry out therapy assessments and follow through with care plans that matter
- Support people to achieve greater independence and mobility in their homes
- Help identify and install assistive equipment that fits personal needs
- Build close links with community teams and residential home staff
- Work independently and handle lone visits with confidence and care
- Step in as a mentor to less experienced team members and student placements
