About the Role
You’ll be stepping into a team with one mission—to help prevent unnecessary hospital admissions. Based at the Royal Gwent Hospital, you’ll support people showing up in emergency care or acute medical units. Every day, you’ll help patients regain control, get back home quicker, and stay safe and supported where it matters most.
Working shoulder-to-shoulder with registered Occupational Therapists, Physiotherapists, and Dietitians, you’ll manage a delegated caseload. The role blends problem-solving, communication, and hands-on care. You’ll support interventions for people managing frailty, cognitive changes, or multiple conditions, all while liaising with families, external partners, and the wider multidisciplinary team.
This isn’t a desk job. It’s fast-moving, people-focused, and mission-driven. And it’s one where every decision you make leaves a mark.
Key Responsibilities
- Deliver therapeutic interventions planned by registered therapists
- Communicate closely with patients, families, and healthcare partners
- Support care plans for patients with complex health needs
- Help reduce hospital admissions and promote home-based recovery

- Contribute to care transitions that feel seamless and empowering
