Requirements
- GCSE-level education or equivalent – with proven English and maths competency
- Relevant NVQ Level 2, 3 or 4 – with hands-on experience in health or social care
- At least 2 years’ senior healthcare experience – ideally in community or cardiac care
- Clear and confident communicator – both written and spoken
- Team player with initiative – able to adapt and remain flexible in a busy setting
- Desire to learn ECG – not mandatory, but a bonus

About the Role
In this Cardiac Assistant Practitioner role, they’re after someone who wants to make a real difference. It’s hands-on, people-first, and purpose-driven. You’ll work with adults navigating complex heart conditions, mostly heart failure, offering personalised care that goes beyond prescriptions.
While the base is a clinic setting, you’ll also head out for home visits or connect digitally if it better suits the patient. The real win? Seeing someone manage their condition better because of the time and advice you gave. You won’t do this alone, either. You’ll work closely with specialist nurses and a consultant cardiologist through weekly multidisciplinary team meetings.

- Deliver and evaluate care plans under supervision
