About the Role
We are seeking a compassionate and experienced Care Coordinator to join our dynamic team. This vital role involves managing the day-to-day operations of our Home Care services, ensuring the delivery of high-quality care and support to our clients. You will develop, implement, and review individualized care plans in partnership with clients and their families, coordinate and supervise a team of care workers, and maintain accurate records and reports in compliance with CQC regulations and company policies.
Key Responsibilities
- Develop, implement, and review individualized care plans
- Coordinate and supervise a team of care workers
- Maintain accurate records and reports
- Conduct audits to high standards, with action plans
- Ensure compliance with regulatory standards
- Supervise staff and ensure efficient running of the service
- On-call duty at least 1-2 weeks a month, in rotation
