About the Role
The Care Home Matron will play a pivotal role in working with the Primary Care Network (PCN) and local care homes to provide enhanced health care services. They will perform advanced clinical assessments, manage complex cases, and coordinate care for residents with long-term conditions. This role is central to reducing unplanned hospital admissions and facilitating hospital discharges. The matron will work closely with community nursing teams and external partners to ensure comprehensive care.
