Job Summary
The Northern Care Alliance NHS Foundation Trust is recruiting two permanent Social Worker positions based at Cromwell House Adult Community Mental Health Team.
Candidates must be registered with Social Work England and have a recognised social work qualification. The role requires a high standard of professional practice, working effectively within a multidisciplinary team. Approved Mental Health Professional (AMHP) status is desirable but not essential.
The role includes regular line management and professional supervision from the team’s Advanced Social Work Practitioner.
Work Schedule:
- Monday to Friday, core hours
- Participation in the Acute Care Pathway outside core hours (weekends and bank holidays on a rota system)
The role is open to newly qualified and experienced social workers. Newly qualified social workers will receive structured support, including the Assessed and Supported Year in Employment (ASYE) programme with mentorship from experienced social workers and an ASYE supervisor.
Main Duties & Responsibilities
- Conduct assessments, support planning, and reviews in accordance with the Care Act 2014.
- Work within the frameworks of the Care Act 2014, Mental Capacity Act 2005, and Mental Health Act 1983.
- Act as a Care Coordinator following the Care Programme Approach (CPA).
- Complete and update risk assessments.
- Work collaboratively in a multidisciplinary team.
- Promote anti-discriminatory practice and uphold the rights of adults with care and support needs.
- Maintain excellent organisation and communication skills.
- Prepare legal standard reports for use in Mental Health Tribunals and Hospital Managers Hearings.
- Fulfill the duties of an Approved Mental Health Professional (AMHP) or Best Interests Assessor (BIA) or be willing to undertake training.
Person Specification
Qualifications
Essential:
- Recognised Social Work qualification (Degree, CQSW, CSS, Dip SW, or equivalent).
- Current registration with Social Work England.
- Willingness to undertake training to become an Approved Mental Health Professional (AMHP).
Desirable:
- Additional qualifications, such as Best Interests Assessor (BIA) certification or other post-qualifying courses.
- Qualified Practice Educator or willingness to undertake training.
Experience
Essential:
- Substantial experience working with individuals with mental health needs.
- Knowledge of the Care Act 2014, Mental Health Act 1983, and Mental Capacity Act 2005.
- Proven ability to build effective relationships with service users and carers.
- Demonstrated good assessment practice.
Desirable:
- Prior experience as a Care Coordinator in community mental health services.
- Experience acting as an Enquiry Lead in adult safeguarding investigations under Section 42 of the Care Act.
- Experience commissioning care packages under the Care Act 2014 and Section 117 of the Mental Health Act.
Knowledge & Skills
Essential:
- Strong understanding of mental health, mental capacity, and human rights legislation.
- Knowledge of personalisation in care planning and the enablement model of health and social care.
- Clear understanding of the role of a Social Worker in a Community Mental Health Team.
- Knowledge of relevant welfare rights legislation.
- Ability to work effectively as part of a multidisciplinary team.
- Strong written communication skills, including the ability to produce legal reports.
- Ability to work with adults to promote independence and well-being.
