Job Summary
Cheshire and Wirral Partnership NHS Foundation Trust is seeking a Children and Young People Mental Health Community Worker to join a Specialist Children and Young People’s Mental Health Team. The role involves providing intensive Tier 4 support and interventions for young people and their families.
Key aspects of the role include:
- Supporting Senior Mental Health Practitioners in delivering safe and effective care.
- Conducting independent work in the young person’s home and other community settings.
- Working under supervision while maintaining a level of autonomy in service delivery.
- Providing care as per the agreed care plan.
- Working variable hours, including out-of-hours and weekend shifts (7 days per week).
Main Duties and Responsibilities
Supporting Young People and Families
- Provide support and interventions that help young people and their carers/families achieve their care plan goals.
- Assist in the development of relapse prevention plans, focusing on young people’s strengths and proactive approaches to maintaining positive mental health.
- Promote young people’s social engagement, health, wellbeing, and age-appropriate independence.
Community Engagement and Partnership Working
- Actively support young people in accessing local community resources.
- Establish and maintain effective relationships with external organizations and agencies.
Care Planning and Crisis Management
- Support service users in care planning and fostering independence.
- Build positive therapeutic relationships to support service users’ decision-making and personal growth.
- Assist young people in creating crisis and contingency plans, where necessary.
- Maintain accurate and up-to-date clinical records.
Person Specification
Qualifications
Essential:
- Assistant Practitioner/Nurse Associate OR NVQ Level 3 qualification in a related field.
- Equivalent training or experience in mental health or children and young people’s (CYP) emotional health and wellbeing at diploma level.
Knowledge and Experience
Essential:
- Understanding of child mental health and its impact on individuals and society.
- Knowledge of common mental health presentations and appropriate support/interventions.
Desirable:
- Awareness of local community groups, services, and demographics.
- Understanding of cultural diversity within Cheshire and Wirral communities.
Work Experience
Essential:
- Experience of working with young people and/or families with complex mental health and wellbeing needs.
- Experience of working effectively within a team.
- Ability to work independently and unsupervised, using initiative.
- Experience in clinical record-keeping, including electronic systems.
- Proficiency in basic word processing and data entry on computers/laptops.
- Ability to prioritize workload efficiently.
Desirable:
- Experience of working as part of a team and independently, seeking support as required.
- Experience of lone working in a community setting.
- Experience of working with children, young people, and families from diverse cultural backgrounds.
