Requirements
- Degree level education or equivalent learning
- NMC Nursing registration kept up to date
- Post registration training that builds clinical confidence
- Knowledge of vulnerable groups and how health systems affect them
- Experience leading services with steady case management
- Comfort with self management when working alone
- Clear communication skills when talking to partners or clients
- Good IT ability with Word Excel and PowerPoint
- Safeguarding awareness with experience applying it

About the Role
This role sits at the heart of rough sleeping outreach across Camden and Islington where you meet people where they are. You build trust by showing up consistently which helps clients open up about what hurts them physically and emotionally. Many avoid healthcare because the system feels overwhelming so your calm presence makes healthcare reachable again. You guide them toward services that support their stability and wellbeing. Some days you walk through familiar routes meeting people in doorways or temporary shelters. Other days you work with partners to understand who needs urgent help. It’s steady relational work and your ability to stay grounded helps reduce health inequalities across the community.
Key Responsibilities
- Engage rough sleepers in their own environment so they feel safe enough to accept support
- Assess physical health needs early which helps prevent crises that lead to hospital care
- Build trust with clients so they stay connected to services that improve long term health
- Work with outreach partners to align plans and avoid gaps that cause missed care
- Manage complex caseloads ensuring each person receives consistent follow up
- Guide clients to community services so they rebuild stability step by step
- Support safeguarding work which protects vulnerable individuals from further harm
- Use clinical judgement to escalate issues when someone’s condition changes
- Help shape service improvements so future clients benefit from smoother pathways
