Job Overview
A vacancy exists for a Band 7 Diabetes Specialist Nurse within the Integrated Cornwall Diabetes Specialist Nursing Team, serving the North/East of the county. This team collaborates closely with community services, secondary care, and primary care providers to deliver seamless diabetes care. Candidates must demonstrate enthusiasm for diabetes care and have prior experience working within a diabetes team.
Key Responsibilities
- Deliver advanced practice services independently, supporting community, primary, and secondary care.
- Manage a patient caseload through nurse-led intermediate clinics and community hospital visits.
- Provide clinical and social support to patients and families.
- Lead the development of evidence-based practices in line with current guidelines.
- Facilitate cohesive multidisciplinary management by ensuring effective communication.
- Educate healthcare professionals and students on diabetes management.
Main Duties
- Provide specialist nursing services and education to individuals with diabetes, their caregivers, and healthcare teams across multiple care settings.
- Manage a patient caseload, acting as an educational resource and promoting communication between health professionals and the voluntary sector.
- Support Cornwall Adult Diabetes Nursing Service in delivering high standards of care alongside the Lead Clinical Nurse Specialist.
- Conduct nurse-led clinics and inpatient visits, offering support to Cornwall Foundation Trust healthcare professionals.

Person Specification
Essential Criteria
- RN Level 1 professional qualification.
- Diabetes specialist qualification (ENB 928 or equivalent).
- Relevant first degree or evidence of progression towards this qualification.
- Significant senior-level experience in an acute or community setting caring for diabetes patients.
Desirable Criteria
- Independent non-medical prescribing course or willingness to undertake this.
