Requirements

  • NVQ Level 2 qualification or equivalent experience in healthcare
  • Confident IT user able to input and manage patient data accurately
  • Strong communication skills for working with patients and multidisciplinary teams

  • Able to travel independently across Walsall during working hours

This role is offered at Band 3 (£24,937 to £26,598 per annum) under the Agenda for Change pay scheme.

About the Role

As a Clinical Support Worker in the Virtual Ward Team, you’ll play a vital part in bringing care to patients where they’re most comfortable — their homes. You’ll support nurses and practitioners by visiting patients, assisting with clinical treatments, collecting data, and helping manage the smooth running of the service.

This role suits someone who thrives in a fast-evolving environment and enjoys connecting with people. You’ll contribute directly to reducing hospital admissions and improving patient recovery outcomes through hands-on care and teamwork.

Key Responsibilities

  • Visit patients in their homes to deliver care and clinical support, ensuring comfort and safety
  • Record and report observations to the team leader to guide treatment decisions
  • Assist in promoting the service, helping families understand its benefits
  • Support data collection and input to maintain accurate patient records
  • Collaborate across teams, ensuring seamless care between hospital and home
