Requirements
- GCSE Grade C or above.
- NVQ Level 4 in Health and Social Care, or willingness to commence this qualification upon appointment.
- Strong communication skills for working with clients, carers, and professionals.
- Understanding of basic human needs—physical, emotional, health, and social.
- Knowledge of social care legislation, including the Care Act 2014 and Mental Capacity Act 2005.
- Ability to manage time effectively and use IT-based care management systems.
- Commitment to multi-disciplinary teamwork and ongoing professional development.
Desirable:
- NVQ/CQF qualification in social or healthcare.
- Familiarity with long-term condition management and risk assessment processes.
Salary Details
This is a Band 6 NHS role offering between £31,049 and £37,796 per year, depending on experience.
About the Role
As a Community Care Worker in the Transitions Team, you’ll help young adults move smoothly into adulthood by coordinating the right care and support. You’ll complete holistic, needs-led assessments, develop care plans, and make sure every individual receives services that truly improve their quality of life.
You’ll work across Baywide Specialist Services, which includes the Review and Insight Team, Sensory Team, and Disability Information Services. While your primary focus will be on the Transitions Team, flexibility to support other teams when needed is part of the role.
This role matters because your decisions shape real lives—helping people live independently and confidently in their communities.
Key Responsibilities
- Conduct holistic assessments and develop tailored care plans that reflect individual needs.
- Manage the transitions inbox and referral process to maintain seamless service delivery.
- Collaborate with multi-disciplinary professionals for effective discharge and ongoing care.
- Ensure compliance with safeguarding and human rights standards.
- Support service users through every stage of care planning and review.
