Requirements

About the Role
As a Health Care Assistant, the role involves participating in the delivery of personalized care to patients within their own homes. This includes routine and acute patient-centered interventions to prevent unnecessary hospital admissions. The role is part of the Stroud Locality Integrated Community Team (ICT), providing essential support to the community nursing teams.
Key Responsibilities
- Delivering a range of clinical care within patients’ homes.
- Supporting the community nursing teams in their routine and acute care tasks.
- Maintaining accurate records of patient care.
- Engaging in regular reflective practice and training sessions.
