Requirements
- NVQ Level 3 or equivalent qualification in Health or Social Care
- Experience in a healthcare or social care setting
- Strong communication and teamwork skills to support both patients and colleagues
- Empathy and understanding of the needs of older adults
- Commitment to person-centred care and a positive, flexible attitude
- Willingness to learn and develop professionally
- Welsh speaking and listening skills at Level 3 (desirable but not essential)
Salary Details
This is a Band 3 position, offering a salary between £25,313 and £26,999 per annum.
About the Role
As a Community Health Care Support Worker within Older Persons Mental Health Services, you’ll help older adults in Merthyr Tydfil live with dignity and independence. Guided by registered nurses and care coordinators, you’ll carry out care plans, monitor wellbeing, and support individuals both in their homes and community settings.
You’ll be the steady presence that helps people feel safe and supported during difficult times. Whether you’re assisting in group activities, helping with daily living, or monitoring physical and mental health, your work will bring real meaning to patients and their families.
You’ll often work independently, making sound decisions while staying connected to a multidisciplinary team. Every conversation, every note, every act of care contributes to someone’s recovery and quality of life.
Key Responsibilities
- Support care plans developed by registered nurses and care coordinators to ensure consistent, quality patient care
- Monitor and report wellbeing through observation and accurate record-keeping, helping the team make informed decisions
- Assist with therapeutic activities that enhance patient engagement and social connection
- Promote healthy lifestyles by sharing information and encouraging participation in wellbeing programmes
- Maintain confidentiality and respect in every interaction, upholding trust with patients and carers
- Collaborate across agencies such as local authorities, voluntary organisations, and housing providers for holistic support
- Prioritise workloads independently while ensuring safety and compassion in all care delivery
